When you need to access your child’s medical records, Coastal Kids has you covered. Patients, parents, legal guardians, and third-party requestors often need records for reasons like continuity of care, legal documentation, or insurance purposes. This dedicated Medical Records page tells you how to request your child’s records easily and securely.

How to request medical records

Patients, parents and legal guardians:

Submit a signed medical records release form (available in English and Spanish) via fax or by bringing it to the clinic. Healthcare providers can also request for our records to be sent directly to their office with a completed authorization form.

Third-party requestors:

All third-party requestors must submit a signed medical records release form (available in English and Spanish). For patients under the age of 18, this form includes the signature of a parent or legal representative. In some cases, additional documents may be required.
